Why skin botox pain comes from how it is delivered, not the drug
Skin botox usually hurts because of how it is delivered rather than the drug itself. To place it in the dermis, a needle is laid shallow and inserted dozens of times across the face about 1 cm apart, and each spot bulges as the drug goes in. So instead of hand injection, fine channels are made in the skin so the drug soaks in evenly. In a study comparing hand injection with microneedling on each side of the face, pore results did not differ between sides. That study followed 30 people for one month, and this is not a pain-free treatment.

People who have had skin botox once often say, "It worked, but it hurt so much I can't do it again." They liked how their oil and pores settled down; getting through the treatment was the hard part.
Some ask whether a different drug would hurt less. Following the pain back, it usually comes less from the drug than from injecting densely across the whole face. This was a long-standing concern, and instead of adding sedation, the delivery itself was rebuilt from the start.
What botulinum toxin does in the skin layer
Most people think of frown lines or forehead lines when they hear Botox. Its ingredient, botulinum toxin, blocks the release of the substance that carries signals from nerve endings. So depending on the tissue it goes into, it can reduce muscle movement or reduce sweating. That is why it is also used for underarm hyperhidrosis and chronic migraine.
Skin botox uses the same drug as forehead or frown-line Botox. The difference is that it is not placed deep into muscle; it is diluted and spread in small amounts through the shallow layer of the skin. Rather than stopping muscles, it acts on oil, sweat, vascular reactions and nerve-driven inflammation in the skin. Expressions stay the same while the skin itself settles down.
Some ask whether pigment improves too. Botox is not a whitening drug that blocks melanin directly. In clinic, as oil, redness and inflammation calm down, the tone sometimes looks clearer or some pigment fades; that is as far as it goes.
A 2024 meta-analysis pooling 10 studies of botox placed in the skin layer (153 participants) found improvements in oil production, pore size, redness, fine lines, texture and elasticity, but no change in skin hydration (Plast Reconstr Surg Glob Open 2024 · PMID 39185380). It combined five randomized controlled trials and five prospective cohort studies, and the authors themselves noted that none reached the information size needed for a firm conclusion. Several authors also disclosed consulting or speaking for botox manufacturers.
Why hand-injected skin botox hurts
Asked what was hardest last time, people who have had skin botox before name the sting and the feeling of the skin swelling as the drug goes in.
Botulinum toxin is a large protein, so simply applying it like a cosmetic does not get enough of it into the skin. It has to be placed with a needle, and for skin botox the drug needs to stay in the dermis just under the surface. The needle is laid shallow and inserted dozens of times across the face about 1 cm apart.
The dermis is dense with nerves, so it stings more than Botox placed into muscle. Each spot bulges as the drug goes in, and with that many needle passes, bruising or swelling can follow. Spreading the drug evenly over a whole face is harder than it looks for the person injecting, too.
Delivering the drug through fine channels instead of hand injection
People who have had Potenza elsewhere often say, "Potenza doesn't hurt much here." It is not a different device; the same device is set lower and gentler to suit the skin. Skin botox was approached the same way: the drug stays the same and the delivery changes.
What the clinic calls Potenza skin botox does not involve injecting across the whole face by hand. Fine channels are made into the skin so the drug soaks in evenly, with preparation to help absorption before and calming care after. The settings and the steps used with it are still being refined as results build up, so they are not all written here.
The first difference people notice is pain. Without dozens of injections, the sting and the swelling feeling as the drug goes in drop considerably. Because the drug is not pushed into single spots, there are no bulging marks at each injection point. Mild redness remains and usually settles the same day or the next.
The drug also reaches spots that are easy to miss with point injections. Making the fine channels itself helps skin renewal, so texture and elasticity care comes along while the skin botox settles oil and pores.
| Hand-injected skin botox | Potenza skin botox | |
|---|---|---|
| How it goes in | Dozens of injections across the face about 1 cm apart | Fine channels let it soak in evenly |
| Face afterward | Bulging marks at each spot · bruising possible | Mild redness · usually settles the same day or the next |
| Included | Drug only | Absorption prep and calming care after · texture care |
A study comparing pore results from hand injection and microneedling
"Doesn't it have to be injected directly to work?" This is the most common question in consultations. One study compared the two methods directly on the same face.
Researchers at Isfahan University of Medical Sciences in Iran enrolled 30 people aged 25 to 60 (29 women) with enlarged pores. Each person had botox injected into the dermis by hand on one side of the face and delivered with microneedling on the other, and the sides were compared one month later in a randomized trial. Three dermatologists who did not know which side was which rated pore improvement using dermoscopy and physical examination (J Cosmet Dermatol 2025 · PMID 40522143).
On both the cheeks and the nose, there was no statistically significant difference between sides, and patient satisfaction was similar. Both methods reduced pore size. If the effect is at the same level, what remains is the pain and burden of getting it.
This study followed 30 people for only a month, and its microneedling method is not the same as the method used here. Potenza skin botox is not yet at the stage of being called a treatment established by long-term, large clinical trials. In clinic, many people notice changes in texture, pores and oil, and quite a few want to repeat it. Still, with a newer treatment the results that accumulate matter more than first impressions, so data are still being gathered on which skin and conditions respond well.
Where skin botox is given by hand and when other treatment comes first
Some people who come for skin botox also mention the jawline or neck. These areas need the muscle under the skin treated as well, so hand injection, which puts a set amount in a set spot precisely, is better there.
If the concern is deep expression lines that fold with movement, Botox placed into muscle fits better than skin botox. When many red, inflamed acne spots are present, skin botox does not come first; the inflammation is calmed down first.
Potenza skin botox is recommended for people who have put off skin botox because of pain, and for those dealing with oil, enlarged pores and rough texture together. It also suits people whose skin flushes easily and looks dull, or who worry about their schedule after treatment because of a face swollen with injection bumps.
Pain with Potenza skin botox and when results appear
Asked "So it doesn't hurt at all?", it is hard to say it is completely painless. There is a slight sting. Many say the burden was much smaller than hand injection, where the whole face is pricked dozens of times and swells. While the numbing cream takes effect, relaxing video and music play in the treatment room.
Skin botox does not work right away; over one to two weeks, oil and pores feel more settled. Skin renewal started by making the fine channels continues for several weeks after that.
Irritation is best kept low while the fine channels close, so makeup is recommended from the next day. On the day of treatment, please also avoid saunas, hard exercise and alcohol.

Areas checked with a magnifier at a skin botox consultation
At a skin botox consultation, the face is examined with a magnifier first. Areas where pores gather, such as beside the nose and the inner cheeks, areas with a lot of oil such as the forehead, and areas where redness comes up are marked separately.
Settings are set differently for each marked area. Thin skin or areas with redness are done lower and gentler, and areas with deep pores are adjusted to suit them. The whole face is not done on one setting.
At the visit, please mention whether you have had skin botox before and where it hurt most. It also helps to say which bothers you most: pores, oil or redness.

Frequently asked questions
QWhy does skin botox hurt?
More of the pain comes from the delivery than the drug. A needle goes into the nerve-dense dermis dozens of times across the face about 1 cm apart, and the skin swells at each spot as the drug goes in.
QIs Potenza skin botox completely painless?
It is hard to say it is completely painless. There is a slight sting, and many say the burden was much smaller than hand injection. Numbing cream is applied first.
QDoes it work as well without injecting?
In a randomized study of 30 people with hand injection on one side of the face and microneedling on the other, there was no significant difference in pore results or satisfaction after one month. It is a small, short study, and it did not test the same method used here.
QWhen do skin botox results show?
Not right away; over one to two weeks, oil and pores feel more settled.
QCan skin botox smooth deep expression lines?
Deep lines that fold with movement are better treated with Botox placed into muscle. Skin botox settles oil, pores, texture and redness.
QCan I wear makeup right after treatment?
Focus on calming the skin on the day, and start makeup the next day. Please also avoid saunas, hard exercise and alcohol on the day of treatment.
