Layered lifting: why the order of the layers matters
People often come back after lifting elsewhere — the result fell short, or it hurt too much, or they had both Ultherapy and InMode and something is still missing. They have one thing in common: the devices were used separately.
Ultherapy works on deep layers such as the SMAS; InMode works higher, in the dermis. Treated separately, each addresses only its own layer and stops there. And firing HIFU into tissue that has not been warmed first costs efficiency for the same number of shots while increasing pain.
Three steps have to interlock in order: warm the tissue, treat the deep layer with HIFU, finish the superficial layer. Each step should raise the efficiency of the next.

The limit of repeating one treatment
Facial descent does not happen in one layer. Dermal collagen declines, subcutaneous fat moves downward, the SMAS loses tension, and the ligaments holding it loosen: all at once. Treat one layer and the others stay as they were.
So however good the Ultherapy, if the layers above it are untouched the result is half of what it could be. Conversely, RF alone refines the surface while leaving the deep descent unaddressed.
What "layered" means here
Layered means building up in order, from the deep layers to the superficial ones.
Step 1: RF or radiofrequency diathermy on the lower face. Heat accumulates in the deeper tissue, laying the ground for what follows. This is the pre-heating step that raises the efficiency of the next.
Step 2: HIFU. Energy is delivered into tissue that is already warm, so it acts efficiently where it is needed and unnecessary shots can be reduced.
Step 3: a pen-type superficial device. Detail in the shallow layers that HIFU does not reach easily is finished with a pen-type handpiece.
Because each step handles a different depth, one programme covers a broad range from the surface to the deep layers.

Why pre-heating matters
The first step is the important one. Warm dough spreads more easily than cold dough. Delivering strong energy straight into cold tissue is less efficient than delivering it into tissue already warmed. When tissue temperature is up, the same energy produces a better response: so pre-heating the deep tissue with RF before HIFU gets the same effect from fewer shots.
- Fewer shots tends to mean less pain
- Energy can be concentrated where it is needed
- Several layers can be addressed in one session
A clinical study in Korean adults over 30 compared HIFU alone on one side with HIFU plus RF on the other, and reported better results on the combined side for pore count, skin elasticity, crow’s feet depth and hydration.
The sample was limited and the protocol is not identical to ours, so the result cannot be generalised directly. Read it as an indication that combining may be complementary.

How the session runs
The three steps run in order in one sitting. It starts with pre-heating the lower face, where the skin begins to feel warm and the tissue prepares for what follows. Then HIFU delivers energy to the deep layer, and finally the pen-type device tidies detail in the shallow layers. It generally takes about an hour, varying by area and by person.
Slight redness or warmth is possible immediately afterwards, but most people return to normal activity without difficulty.

Who this suits
People who have had Ultherapy or InMode separately and found the result short; people who hesitated over lifting because of pain; people who want several layers addressed at once; people who want lifting and volume together.
Not everyone gets the same steps at the same intensity. Korean faces tend to carry thicker fat over the cheekbone and fuller midface volume, so the same energy produces different results in different people. The structure and the pattern of descent are assessed first, then the intensity and area of each step are adjusted: longer pre-heating for one person, a greater share of HIFU for another.
Aftercare
The effect of a lifting treatment does not end on the day. It appears gradually as collagen regenerates, so aftercare affects the result.
- Avoid saunas, hot baths, hard exercise and heavy drinking for a few days
- Be strict about sun protection. UV on irritated skin slows recovery
- Moisturise well to settle the skin barrier
Frequently asked questions
Is Ultherapy alone not enough?
Ultherapy is strong at the deep SMAS but does not also handle the shallow layers and surface detail above it. A layered approach pre-heats with RF, treats the deep layer with HIFU, then completes the shallow layer.
Does it hurt less?
Most lifting pain comes from delivering strong energy directly into cold tissue. Pre-heating first means the same effect from fewer shots, which tends to reduce the pain. Tell us at consultation if you are sensitive to pain.
When do results appear?
It varies with skin condition and degree of ageing. Change generally appears gradually and continues over the months during which collagen regenerates.
Can I go back to normal life the same day?
Yes. Slight redness or warmth is possible but usually settles quickly. Avoid saunas and hard exercise on the day.
How often?
Lifting holds better with periodic treatment than as a one-off. The right interval depends on skin condition and the degree of ageing.
