Ultherapy, Cerf, InMode: how energy-based lifting devices actually differ

The question I hear most from patients worried about sagging is "which lifting treatment should I get?" You may know the device names — Ultherapy, Shurink, Cerf, InMode. Rather than memorising names, it helps to know which layer the energy reaches and how.

How lifting devices are classified

Energy-based devices (EBDs) for lifting divide broadly by how the energy acts and how deep it goes.

  • HIFU (high-intensity focused ultrasound) · Ultherapy Prime, Shurink Universe
  • RF (radiofrequency) · further divided into monopolar and bipolar

Where each kind of energy lands. HIFU makes points in the deep layer, monopolar RF heats broad and deep, bipolar RF stays in the shallower layers.

HIFU: deep, strong, point by point

Ultherapy Prime and Shurink Universe are typical HIFU devices. They focus ultrasound energy on a single point, creating thermal coagulation points in the deep layers at roughly 60–70°C. The targets are the deep dermis and the SMAS layer.

The SMAS is the fibrous network beneath the skin connected to the facial muscles. It is the same layer pulled in a surgical facelift. HIFU acts on this supporting structure, not on the skin surface.

RF: heating a plane, not a point

Radiofrequency generates heat through electrical resistance, mainly in the dermis and subcutaneous fat, warming a wide area fairly evenly. This triggers collagen contraction, increased blood flow and fibroblast activation.

Monopolar RF (Cerf) carries current deep and wide, firming the skin itself. Bipolar RF (InMode) concentrates energy in shallower layers, trimming fat and refining the line.

So which device?

Facial ageing is never one problem.

  • Sagging · the SMAS and retaining ligaments
  • Loss of firmness · the dermis
  • Volume change · the fat layer

That is why "which device is best" has no single answer. Sagging points to HIFU; firmness and texture point to monopolar RF; jawline contouring points to bipolar RF. In practice, most faces have more than one change, so devices are combined after diagnosis.

Sagging comes from the SMAS, loss of firmness from the dermis, a change in volume from the fat layer. Which layer it is decides the device.

What is the difference between Ultherapy and Shurink?

Both are HIFU devices with the same principle. They differ in energy delivery characteristics and tips, so the choice depends on skin thickness and pain sensitivity.

Are Cerf and Thermage the same?

Both are monopolar RF in the same family, aiming heat at the deep dermis for collagen remodelling, with differences in tips and energy profiles.

Is InMode alone enough for lifting?

InMode is strongest at shallower layers and fat contouring. If sagging originates deep, HIFU or monopolar RF should be considered; for jawline refinement alone, InMode can be enough.
